From the findings of the non-partisan Government Accountability Office report:
"It was possible to alter the files that define how a ballot looks and works so that the votes for one candidate could be recorded for a different candidate." Numerous sworn statements and affidavits assert that this did happen in Ohio 2004.There's more here.

Why can't we all be assigned an individual voter number, go and vote, or vote online, and be able, after an election, to check a list of all voter numbers to make sure our particular vote was registered. Is it because it wouldn't be private or anonymous? Is it actually anyways? We have to come up with a better system than the antiquated voting machine punch card thing. It was so obviously fraud in 2000 and then AGAIN we let them get away with it in 2004!
